P.R.E.P. Veterinary Physiotherapy has disclosed to me the nature of holistic and complementary therapies and rehabilitative veterinary medicine.

I authorize P.R.E.P. Veterinary Physiotherapy to use the recommended rehabilitative and complementary treatments on my animal. I understand that P.R.E.P. Veterinary Physiotherapy’s recommendations and treatments of my animal constitute no guarantee of results. I further understand that P.R.E.P. Veterinary Physiotherapy’s recommendations and treatment are based, to some degree, on the information that I supply and my willingness to complete the prescribed exercises and other modifications pertaining to environment, food and veterinary referrals.

In consideration of the foregoing, I waive and release any and all rights, claims, and causes of action I have or may have against P.R.E.P. Veterinary Physiotherapy and its affiliates, employees, consultants, representatives, that may arise as a result of any of P.R.E.P. Veterinary Physiotherapy recommendations or treatment of my animal, including, but not limited to any hydrotherapy, therapeutic exercise, or other physiotherapy treatments or care

Cancellations, Late Arrivals, and Missed Appointments

We request that in situations requiring appointment cancellation we receive at least 24 hours’ notice. This allows us to provide that appointment time to another animal in need. We reserve the right to charge a cancellation fee of R600 should a confirmed booking be cancelled less than 24 hours prior to the appointment. Appointments for late arrivals will be shortened to meet the scheduled completion time unless time is available to enable an extension. Appointments arriving more than 10 minutes late may need to be rescheduled to avoid disrupting the appointments of other patients. We understand that emergencies arise and appreciate you keeping us informed of unforeseen circumstances.

Financial Payment

Payment is due in the form of cash, or EFT on the day of treatment unless otherwise arranged. Monthly payments are due at the end of the month and no later than the 7th of the following month. P.R.E.P. Veterinary Physiotherapy reserves the right to charge a late bill fee of R150 per day overdue after the 7th of the month. Failure to make payment will result in legal action. All services purchased and fulfilled are non-refundable. We will be able to refund the price for any unused services due to medical issues or unforeseeable circumstances. Products purchased at P.R.E.P. Veterinary Physiotherapy may be returned for a full refund if they are unopened and returned within 30 days of purchase. Prices are subject to change.

Multimedia Release

P.R.E.P. Veterinary Physiotherapy often records photographs and videos of patients for the purposes of education, promotion, or advertising.

General

Where an animal shows symptoms which may require veterinary attention (i.e. obvious signs of lameness or illness), it may be necessary to delay treatment until veterinary advice has been sought.

For cases (any animal) requiring more regular treatments i.e. tendon or ligament injury, neurological disorders etc an individual pricing structure will be discussed. 

For visits to multiple animals at one location or a location ranging between 5-10km a discount may be available, to be arranged before treatment date.

On the day of your appointment, please ensure that your animal is clean and dry to allow for accurate assessment of musculature.
